Developing the Individualized Service Plan (ISP). This program, designed for persons who develop or supervise the development of the ISP required for residents of Assisted Living Facilities licensed by the Virginia Department of Social Services. Objectives: 1. Read & interpret the Uniform Assessment Instrument (UAI). 2. Identify the Virginia licensing standards and regulations governing the completion and revision of the UAI and ISP. 3. Explain what is meant by the phrase, "Significant Change". 4. Discuss how ISPs can promote teamwork. 5. Identify resident needs from the UAI and other assessment resources. 6. Explain the difference between a diagnosis and a need. 7. Discuss ways to individualize an ISP. 8. Develop a goal oriented ISP. - The presentation uses the Virginia Department of Social Services 2018 Curriculum. - Approved by NAB/NCERS for 4 clock hours. Please Note: THE PRIVATE PAY UAI TRAINING is REQUIRED BEFORE TAKING THE ISP TRAINING. (This training can be accessed at the DSS website)
